Pre-Employment Requirements:
- Drug test
- Criminal Background check – over three years since prior conviction
- TB skin test: Must have a test or provide proof of one within the past year.
- Must have a valid driver’s license, reliable transportation and current automobile insurance.
- Must have a minimum of a bachelor’s degree in social work, psychology or related field.
- Prefer 2 years’ experience in chemical dependency.
Position Summary: Access, coordinate and monitor services to assist the client in becoming stable in the community and reaching the highest level of potential with alcoholism or substance abuse.
Skill Sets: Demonstrate good oral and written communications. Maintain a supportive and encouraging working relationship with co-workers and clients. Participate in the initiation, coordination, and implementation of the referral plan. Follow-through, monitor, and evaluate activities related to the recovery of the client. Must have basic computer skills.
Physical and Environmental Demands: Visual acuity and manual dexterity are needed for the required paperwork. Frequent talking and hearing are required when communicating with co-workers, families, and other service providers. Physical demands are light with walking, standing, and occasional stooping.
Job Responsibilities:
- Able to provide customer service to personnel, persons served, families, and stakeholders.
- Help maintain the health, safety, and welfare of personnel, persons served, and stakeholders.
- Ensure the agency is operating under CARF accreditation standards.
- Ensure that state and federal licensure and regulations are being adhered to.
- Individual assessment of
- Basic needs such as shelter, food, clothing, entitlements, etc.
- Specialized needs such as substance abuse treatment services, rehabilitation, social opportunities, vocational and employment capabilities, and psychiatric services.
- Develop and facilitate life skills learning to help clients prepare for post-program living.
- Determine clients’ needs for childcare and transportation in order to access services.
- Provide ongoing contact with community agencies to develop additional resources and coordinate services.
- Develop and monitor a treatment plan for each client in the assigned caseload.
- Advocate for the client.
- Assist clients with the problems they face in early recovery, especially legal issues, lack of housing, and unemployment.
- Complete necessary initial documentation with client including Assurance of Services, Needs Assessment, and Service/Treatment Plan.
- Complete 5 monthly contacts and document with client- 3 of which must be face to face.
- Complete Monthly Summary for each client.
- Maintain a caseload of up to 25 clients.
- Willing to have on-call responsibilities.
- Oversee the application process to obtain food stamps for clients and insurance recertification.
